General
The well-shaped breast of a woman embodies symbolically womanhood and attractiveness and sexuality. Whereby the ideal breast shape of course depends on the current fashion (Rubens, Twiggy and so forth) and the different cultural preferential. With ideal breast I mean cupsize B-C which is mainly presented in the media and by TV stars. Breast surgery is possible from age 18 on, i.E. when the mammary gland is fully developed. Deviation of the breast shape can either be congenital or developed (i.E. pregnancy), oversized or too small or of different size. The surgical techniques can tackle and do justice almost any questioning. Depending on the physical condition enlargements with round or anatomical implants or even combination of enlargement and tightening can be taken into consideration. In the meantime the controversial discussion on silicon implants has finally come to a rational result and concluded that the modern and high-quality silicon implants are safe for breast augmentation. Obviously this requires the exclusive use of high-quality implants from leading manufacturers. Capsular contracture alone cannot be excluded as a risk factor. In my experience 1-2 % of the patients are affected and means a hardening of the breast through additional conjunctive tissue and scaring around the implant. Not in all cases and depending on peculiarity another surgical intervention will then be necessary. In our consultation I will explain in detail the different implants (size and shape) as well as surgical technique. The scar can be in the crease below the breast, around the areola or under the armpit. The implant can be placed above or under the muscle. Hereby I weigh up the pros and cons with regard to the individual patient thoroughly in order to give a sensiblere commendation. In terms of size of the implants I am basically in favour of harmonic body-proportions.
Surgical techniques
The surgery is performed with only one incision either from the crease below the breast, around the areola or under the armpit. Now a pocket for the implant will be moulded. When little skin/gland tissue is available I prefer locating the implant underneath the muscle, if enough tissue is present to cover the implant it will be positioned above the pectoral muscle. At the same time the sagged tissue will be tightened whereby the extent of the scars depends on the skin surplus. Breast augmentation surgery usually takes about one hour to complete (without tightening at the same time) and is done under general anaesthesia. The small incision (4 cm) will be closed with a cosmetic stitch.
Aftertreatment
A week later I will change the first bandage. A sport-bra will be worn consistently over a period of 4 weeks. The patient should not strain the body for the same amount of time to ensure a healing of the wound without complications. Thereafter one can do the same movements as before the surgery. No threads need to be taken off as they dissolve by themselves. The scar should be treated with Bepanthen ointment (apply thinly). The patient can be released 1 day after thesurgery. A check-up should follow after 3-4 weeks and 6 months after the surgery. The result will then be adequately.
